The ICB’s Frailty Team has been working in partnership with clinical experts from across the system to develop an animation to promote annual health checks for dementia patients. The short animation focuses on the benefits for patients, as well as their loved ones, by showcasing the different kinds of support they can access via the annual health check, which is delivered by general practice.
